Nurse - Home Care Nurse, with Experience in Care Coordination, in Renens VD

Within Vivràdom, a private home care organisation, we offer an exciting position alternating between two missions for a confirmed nurse:

Within a centralised nursing team - mornings or afternoons

• Ensure the reception, assessment and coordination of home care requests, guaranteeing care adapted to the clients' needs.
• Organise continuity of care, especially during returns home after hospitalisation, in collaboration with interdisciplinary teams and the care network.
• Support and advise field teams, ensure clinical follow-up of situations and oversee the quality of services as well as record keeping, in collaboration with a case manager.

With patients/clients at home - mornings or afternoons

• Contribute to maintaining clients at home by promoting their autonomy, prevention and health promotion.
• Carry out health assessments and defined care.

Working hours as a Nurse

During the day, on weekdays

Some weekends in the field, some weekends and evenings on telephone duty

Your experience as a nurse

Bachelor's degree in nursing

Experience in Coordination, Organisation and Planning

Confidence in clinical assessment, prioritisation and decision-making

Ability to manage rapidly evolving health situations and complex cases

Autonomy, responsiveness and a sense of responsibility
