Home Care Social Worker 70%

For one of our clients, we are looking for a Social Worker in home care 70% H/F. You hold a Bachelor HES-SO / HETS-Geneva in social work or deemed equivalent. You have very good knowledge of the Geneva socio-health network. You must have at least 2 years of successful experience in supporting vulnerable, excluded or marginalized populations. You will manage requests from professionals in the Geneva socio-health network and respond quickly or direct them to the appropriate service. Conduct, with the nurse, a socio-health assessment by providing social expertise, propose short-term interventions while respecting the legal framework. Support individuals and their families in their social and administrative processes; Write a precise report of each intervention, including materials provided and retained.
